How to use this resource
- Identify the lens and eye. Keep left and right records separate even when the two dates happen to match.
- Copy the opened date and the replacement date established by the instructions for your lenses and your eye-care professional.
- Note where the date came from. If the instructions are unclear, ask your eye-care professional instead of selecting an interval from this sheet.
- When a lens is replaced, start a new row. Keep the prior row as history rather than silently reusing it for the new lens.

A recorded date is not a safety assessment
